What You Can Report :

Adverse Event (AE): Any undesirable or unexpected sign, symptom, or medical condition that occurs during the use of a medicine, regardless of whether it is considered to be related to the medicine.
Medication Error: Intentional or unintentional use of a medicinal product in a way that does not comply with the approved instructions for use.
Off-Label Use: Intentional use of a medicine in a manner not specified in the approved product labeling.
Misuse: Intentional use of a medicinal product over an extended period, intermittently, or in excessive amounts, potentially causing harmful effects on physical or mental health.
Lack of Efficacy: Failure of a medicinal product to produce the expected pharmacological effect under approved conditions of use. Example (vaccine): Receiving a vaccine to prevent a disease but still developing the disease.
Effect on Professional or Non-Professional Activities: Any impact on the user as a result of the medicine during professional or non-professional activities.
Overdose: Use of a medicinal product in amounts exceeding the approved single or daily dose, including effects resulting from cumulative overdosage.
Unexpected Beneficial Effect: Any unanticipated positive outcome observed after using a medicinal product, which was not intended, studied, or documented in clinical trials.
Medical Error: Unintentional mistakes made by healthcare professionals, patients, or users during prescribing, preparing, or administering a medicinal product.
Suspected Transmission of Infection: Possible infection associated with the use of a medicinal product, whether or not a causal relationship is established.
Use During Pregnancy or Lactation: Any exposure to a medicinal product during pregnancy or breastfeeding, including potential effects before conception (e.g., if the mother or father is taking the medicine during conception or early pregnancy).
